New Clients
If you are new to Boon Plan Management, you can sign up using our Participant Registration Form.
In addition, the following forms are also recommended, but optional. If you are unsure, please call or email us to discuss your needs.
Please note: If you have a PACE plan, you are required to endorse us as your plan manager so that we can access your funding and make payments on your behalf. Please contact your LAC Planner (their details can be found in your NDIS Plan document) and advise that you would like to endorse us as your plan manager. The information that you will need to provide is in the Provider Information for PACE Endorsement below.

You are able to provide updated information to us at any time using the forms below. If you are unsure if you need to complete updated forms, please call or email us to discuss your needs.
Incident Report Form
Please use this form if you would like to notify us of an incident that we need to address.
NDIS Provider Complaint Form
Please use this form if you would like to notify us of a complaint you have with an NDIS provider, NDIA employee or representative, Local Area Coordinator (LAC), or other professional that you have had contact with in relation to your NDIS plan.
Boon Client Feedback Form
Please use this form if you would like to notify us of an area that we could improve in, or if you'd like to give us a compliment and tell us about something we are doing well. You can submit responses anonymously and all feedback we receive is used to develop our processes and improve our services.
Provider Information for PACE Endorsements
If you would like to use Boon Plan Management as your plan manager for your next plan, you can endorse us during your planning or implementation meeting. This will make setting you up in our system much quicker and allows us to begin processing payments almost immediately. You can download the document below and share it with your NDIS Planner or Local Area Coordinator (LAC). It contains all the information about us that is needed to have us endorsed as your preferred plan manager.
Provider Invoicing Guide
This document can be downloaded and shared with your providers and contains all the information they need to ensure that their invoices are NDIS compliant and our contact details to help make claims and payments as simple as possible.
